Adult Initial Visit (45-minute appointment): $95
Adult Follow Up Visit (30-minute appointment): $85
Youth Office Visit for patients 18 and younger:
Initial visit: $75, follow up visit: $65
Dr. Michelle is a non-participating provider with all insurance plans. This means payment is due at time of service. A receipt may be provided to you for submission to your insurance company. This however is not a guarantee of reimbursement.
Cancellation Policy:
$50 fee for Missed Appointments
Please understand that appointment times are limited. If you must cancel your appointment, I respectfully request 24 hours notice. Of course, if you're not feeling well or concerned you have COVID; please call to cancel. I need to set some boundaries to prevent "no shows". These gaps in my schedule are hard to deal with because it's time I could have given to another patient. I will always be fair and reasonable in my approach, but I also need to give it some structure!
Credit Card to Secure an Appointment
I'm requesting all patients have a credit card on file to secure an appointment. This information is stored securely in my patient database and will make the checkout process easier.
