Adult Initial Visit: $120
Adult Follow-Up Visit: $95
Adult Prepaid Packages: 6 follow-up visits for $525. 10 follow-up visits for $825.
Youth Office Visit for patients 18 and younger: Initial visit: $90, follow-up visit: $75
Payment is due at the time of service. A receipt will be provided to you for submission to your insurance company. This, however, is not a guarantee of reimbursement.
Cancellation Policy
$50 fee for Missed Appointments
Please understand that appointment times are limited. If you must cancel your appointment, I respectfully request 24 hours’ notice.
Credit Card on File
All patients must have a credit card on file to secure an appointment. This information is stored securely in my patient database and will make the checkout process easier.
